Enroll a Student

The William S. Hart High School District requires the following information to enroll a student into our high school:
1. A parent/guardian must be present when enrolling a student.
2. Proof of residence - We will accept any two current utility bills with the name and address of parent/guardian to verify residence.

3. Original Birth Certificate/Passport

4. Withdrawal Grades - for mid-term enrollees

5. Unofficial Transcript or copies of all report cards.

6. Most recent IEP if Special Ed student

7. Up-to-date Immunization Record - NO EXCEPTIONS

8. Mantoux Tuberculosis Test and Varicella Immunization(or doctor’s verification of Chicken Pox)-Only required if student has never attended a California school before. These services may be provided by any doctor or clinic or at:

9. Guardianship Forms - If student is not living with a parent or legal guardian, A Caregiver Affidavit will be provided and must be completed in our office. Caregiver must present picture I.D. along with proof of residence. Legal guardians must show court documentation.

PLEASE SIGN ALL ATTACHED FORMS AND RETURN TO THE REGISTRAR. AFTER ALL PAPERWORK HAS BEEN PROCESSED WE WILL SET UP AN APPOINTMENT WITH THE COUNSELOR TO SCHEDULE CLASSES
